Aide Health is tackling this problem by offering a digital platform to help patients manage their conditions more effectively.
Its platform provides patients and clinicians with a mobile app that gives personalised advice, tracks health data, and uses AI-driven daily conversations to support self-management. Ultimately helping patients stay on top of their care and making it easier for them to follow treatment plans and communicate with their healthcare providers.
It's seeing the success of its efforts, too. Recently expanding to London, it now serves around 300,000 patients across four primary care networks. Partnerships with NeoHealth Primary Care Network in West London mark a key milestone for Aide Health. With proven success in reducing hospital admissions, the company has very much set the groundwork to become a key player in the digital health space.
